Please print it off and send to us. We use this information to keep track of where our materials are being broadcast for follow-up and advertising purposes. Once we have received this form, you can obtain downloadable audio versions of our material by contacting client-care@amazingdiscoveries.org

Only AD produced materials may be aired, and these must be aired in their entirety. Also we request that you send a monthly report outlining which programs you aired and when.
